Individualized Care Plan
You and your loved ones will get guidance and support from your primary care provider and other members of your care team. Your treatment plan may include:
- Blood sugar monitoring
- Diabetes education program
- Lifestyle changes, such as following a diabetes-friendly meal plan and getting enough exercise
- Medications
Diabetes Nutrition Counseling
Learn which foods help control blood sugar and which foods to avoid to manage diabetes through your diet. You may benefit from one-on-one or group sessions with a registered dietitian at one of our hospitals.
Learn to Use Insulin
Insulin is a hormone used by the body to control blood sugar levels and give you energy. But if you have diabetes, your body isn’t able to use or make insulin normally. Insulin therapy can help you keep your blood sugar at a normal level.
Insulin Equipment
Depend on our staff to help you take insulin as recommended by your doctor. We’ll help you learn to safely use and store the devices and equipment you need to take your medication. You may have training to use:
- Insulin pen – Disposable needle you use to inject the recommended amount of insulin from a cartridge into your body
- Insulin pump – Small device that sends insulin into the body throughout the day through a catheter, a thin and hollow tube placed under the skin
