Online Records Request
Patients of Adventist HealthCare Fort Washington Medical Center, Shady Grove Medical Center, White Oak Medical Center and Rehabilitation may request a copies of their medical records online using our secure platform that verifies your identity by asking for a photo of your driver’s license, which can be taken via webcam or smartphone. There is no charge to use this service.

Our offices are open from 8:00 am to 4:30 pm Monday through Friday (except holidays). Mailed medical record requests may take approximately 5-10 business days to be processed after receiving your request.
