- Filling out this form does NOT register you for a clinic, add you to a waitlist or guarantee vaccination.
- As capacity opens at our vaccination clinics, Adventist HealthCare will email all subscribers with alerts on new locations, dates and times.
- You must be eligible under the state of Maryland’s current phase criteria to be vaccinated.
- Please do not call our hospitals. As a subscriber, you will receive an email alert as soon as there is availability.
- Vaccine supply is extremely limited at this time and demand is such that we expect registrations to fill very quickly each time they open. We kindly ask your understanding and patience as we work to add more availability.
- Please do not walk up to an Adventist HealthCare facility for vaccination without an appointment.
